📋 Description

• Deliver direct support per ISP/BSP and other plans
• Facilitate structured community-based activities
• Transport individuals and coordinate community outings
• Support participation in events and ISP outcomes
• Document services and ensure health, safety and dignity
• Promote community integration and self-determination

🎯 Requirements

• 1+ years in human services or related fields
• Valid driver’s license and vehicle with auto insurance
• Must be at least 18
• Provide references
• Attend two weeks of paid orientation/training
• Good communication and team-oriented
