Home Health Visits Registered Nurse

📋 Description

• Provide skilled nursing in the home as part of a multidisciplinary team
• Conduct head-to-toe assessments and identify changes in patient condition
• Manage acute and post-acute care (CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and independently manage urgent clinical situations in the home
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation

🎯 Requirements

• A current nursing license in good standing in Massachusetts
• Graduation from an accredited nursing program with transcript/diploma
• At least one year of current or recent hands-on experience with adult/geriatric population in acute
