• RN or LPN to serve patients in their home and community
• Conduct annual RAI and GHE assessments for Medicaid clients
• Collaborate with clients, caregivers, and teams
• Document findings to meet state requirements
• Support Medicaid-funded in-home services reauthorization
• Part-time visits with flexible scheduling

🎯 Requirements

• Current MO RN or LPN license
• Current CPR certification
• Strong organizational and communication skills
