Home Care RAI Visits - Registered Nurse or Licensed Practical Nurse

📋 Description

• Conduct annual RAI and GHE assessments for Medicaid clients in-home
• Evaluate functional abilities, health status, and care needs
• Document findings and ensure state compliance
• Collaborate with clients, caregivers, and interdisciplinary teams
• Support Medicaid reauthorization for in-home services
• Monitor changes in client status and identify care needs

🎯 Requirements

• Current MO RN or LPN license in good standing
• Current CPR certification
• Strong organizational and communication skills
