Job Description
📋 Description
- Provide direct support to individuals with intensive behavioral and mental-health needs using
- Support individuals per behavior and treatment plans, crisis-prevention strategies, de-escalation
- Maintain a clean, safe, structured living environment; assist with daily living skills, tasks
- Document services, observations, health concerns, and activities using electronic systems (T-logs
- Monitor behavior changes, identify escalation indicators, and communicate with supervisors and team
- Support family and personal contacts, assist with appointments and medication administration tasks
🎯 Requirements
- High school diploma or GED
- 3 years minimum of experience with individuals with chronic and severe mental illness OR a degree
- Must be at least 18 years of age; some programs require 21+
- Valid driver’s license and proof of auto insurance
- Basic computer skills and ability to use electronic documentation systems
