Job Description
📋 Description
- Provide home health nursing visits in designated geographic area (Schuylkill County).
- Follow care plans and administer medications and wound care per patient needs.
- Monitor client conditions and report changes to the manager.
- Document observations, interventions, and evaluations using EMR/tablet.
- Coordinate case management and follow up on doctors' orders.
- Deliver 1:1, compassionate care with strong clinical judgment.
🎯 Requirements
- Current PA Registered Nurse license.
- Minimum 1 year of recent clinical (medical/surgical) experience.
- Home health experience preferred but not required.
- Graduate of an accredited nursing program; ability to work independently.
- Strong interpersonal skills and solid computer/EMR skills.
- Willingness to travel to cases as assigned.
