LPN - Transition Team

📋 Description

• Support transition of care from hospital to home care in Chester, Delaware, and Montgomery Counties.
• Deliver quality nursing care to infants, children, and adults in home settings.
• Provide extensive training to ongoing BAYADA care team.
• Day and overnight shifts available (Monday through Sunday).

🎯 Requirements

• NICU, PICU, ICU, or home care trach/vent experience
• Pediatric trach/vent experience required
• Hold current CPR certification
• Ability to travel throughout Chester, Delaware, and parts of Montgomery County
• Ability to work a consistent 40-hour per week schedule
• Weekend availability (at least four weekends per month)
