• Provide direct support to individuals with intensive behavioral and mental health needs using
• Support plans, crisis-prevention, de-escalation, and manual restraint as trained.
• Maintain a clean, safe living environment; assist with daily living activities and community
• Accurately document services and observations using electronic systems (T-logs, Therap).
• Monitor behavior changes and escalate issues with supervisors as needed.
• Collaborate with family, staff, and external partners to meet needs.
