📋 Description

• Evaluate patients’ functional status in home settings
• Develop individualized OT treatment plans
• Assess cognitive, psychological, and motor performance
• Assess home hazards for safer living
• Identify equipment needs and environmental adaptations
• Revise care plans with nursing in the multidisciplinary team

🎯 Requirements

• Licensed Occupational Therapist in Hawaii
• Willingness to travel within assigned territory
• Graduated from accredited OT program
• 1 year OT experience preferred
