📋 Description

• Provide skilled nursing and rehabilitative care to clients at home as part of a multi-disciplinary
• Perform head-to-toe assessments and identify subtle changes in patient condition.
• Manage acute and post-acute care needs (CHF, COPD, wound care, IV therapy, post-surgical care).
• Triage and independently manage urgent clinical situations in the home setting.
• Conduct medication reconciliation, patient teaching, and safety monitoring.
• Complete comprehensive wound assessment, care, and documentation.

🎯 Requirements

• Current MA nursing license in good standing.
• Graduate of an accredited nursing program.
• At least 1 year of hands-on medical experience with adult/geriatric populations in relevant
