Occupational Therapist, OT - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status, including muscle function, endurance
• Assess cognitive, psychological and motor aspects of performance and develop individualized
• Evaluate home environments for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations to improve outcomes.
• Continually assess and revise the OT care plan and collaborate with nursing in the

🎯 Requirements

• A current Colorado Occupational Therapist license.
• Prior home care experience preferred, but not required.
• A minimum of one year of recent OT experience.
• Graduation from an accredited OT program with transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem-solving skills; solid EMR skills.
