📋 Description

• Make home visits to clients in designated geographic territories.
• Provide rehabilitative care and skilled nursing in a home health setting.
• Assess mobility, gait, balance, and safety; develop care plans with therapists and nurses.
• Collaborate with multidisciplinary team to support independent living at home.
• Document observations and interventions using EMR tools.
• Support patients in completing prescribed therapies to improve outcomes.

🎯 Requirements

• Current New Hampshire PT license.
• Minimum one year of recent PT experience.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Experience with EMR systems and documentation.
• Education from an APTA/CAAHEA-accredited program.
