Senior Living Physical Therapist

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, transfer status, balance, strength
• Develop and implement individualized care plans: manual therapy, gait training, balance, and
• Assess and revise PT care plan; collaborate with nursing in multidisciplinary care plan.
• Educate clients and families to promote health, safety, and independence.
• Document observations and interventions using electronic medical records (EMR) with accuracy.

🎯 Requirements

• Current New Hampshire Physical Therapist license.
• Minimum of one year of recent PT experience.
• Graduate from APTA/CAAHEA-accredited program (per school transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
