Licensed Practical Nurse (LPN) - No Weekends

📋 Description

• Provide individualized health care management for residents
• Perform routine site visits, vitals, assessments, med counseling, progress notes
• Serve as health resource and train staff in medical topics
• Audit and review medications
• Ensure compliance with state requirements and policies
• Schedule: FT Mon-Fri, some travel; on-call 1 week/mo

🎯 Requirements

• Current Indiana LPN license
• 1-2 years experience with disabilities or long-term care preferred
• Flexible, dependable, team player
• Strong verbal/written communication
• Ability to work independently and efficiently
• Strong organizational and computer skills
