Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Assess mobility, range of motion, ambulation, strength, balance, and safety.
• Implement wound care and preventive interventions with evidence-based dressings.
• Develop and adjust individualized care plans including gait and balance training.
• Collaborate with nursing and multi-disciplinary team on care plans.
• Educate clients and family members on rehabilitation and safety.

🎯 Requirements

• PA Physical Therapist license in good standing.
• Minimum one year of recent PT experience.
• Graduate from APTA/AAAHEA-accredited program with diploma/transcript.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient computer skills; EMR experience preferred.
