Registered Nurse-Home Health Visits

📋 Description

• Provide skilled nursing and rehab care at patients' homes
• Perform comprehensive assessments and detect condition changes
• Manage acute and post-acute care needs (CHF, COPD, wounds, IV therapy, post-surgical care)
• Triage and independently handle urgent clinical situations
• Medication reconciliation, patient teaching, and safety monitoring
• Wound assessment, care, and documentation

🎯 Requirements

• Current nursing license in good standing
• Graduate of an accredited nursing program
• At least 1 year of adult/geriatric clinical experience in relevant settings
