Registered Nurse, Evenings

📋 Description

• Per Diem On-Call RN for evening home health coverage
• Provide in-home skilled nursing, SOC admissions as needed
• Operate in Somerset, Union, Middlesex, Monmouth Counties (NJ)
• Deliver patient-centered care with a clinical team support
• Respond after-hours patient needs within coverage area
• Document care using EMR and follow physician orders

🎯 Requirements

• Current RN license in New Jersey
• Minimum 1 year home health nursing experience
• Experience with OASIS documentation and SOC visits
• Strong assessment, clinical decision-making, communication
• Comfort with EMR; HCHB experience a plus
• Reliable transportation to travel across counties
