Registered Nurse, Evenings

📋 Description

• Per Diem On-Call RN for evening home health coverage
• Provide in-home skilled nursing visits in Somerset, Union, Middlesex, Monmouth
• Occasional Start of Care admissions as needed
• Deliver patient-centered care with a clinical team support
• Respond to after-hours patient needs within coverage area
• Document care and EMR entries during visits

🎯 Requirements

• Current RN license in New Jersey
• Minimum 1 year home health nursing experience
• Experience with OASIS documentation & SOC visits
• Strong assessment, clinical decision-making, and communication
• Familiar with EMR systems; HCHB is a plus
• Willingness to travel across Somerset, Union, Middlesex, Monmouth
