Speech Language Pathologist, (SLP) - Home Health

📋 Description Make home visits to clients in designated geographic territories. Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients Develop a treatment plan based on evaluation, physician orders, and goals. Revise care plans and participate in multidisciplinary team care plans as appropriate. Educate patients, families, and care representatives in rehabilitative care and activities for Document observations and interventions using an EMR/tablet system. 🎯 Requirements Current North Carolina Speech Language Pathologist license. Certificate of Clinical Competence in Speech (ASHA CCC) or equivalent qualifications toward Completed education/work experience required for licensure and ongoing certification process.
