📋 Description Perform orientations and assess new patients for our program. Train caregivers to ensure high-quality in-home care. Conduct home visits and contribute to ongoing patient care plans. Coordinate with caregivers, patients, and staff; travel within GA. Engage in interdisciplinary meetings and provide case updates as required.
