📋 Description Provide skilled nursing and rehabilitative care to home health clients as part of a Perform comprehensive head-to-toe assessments and identify changes in patient condition. Manage acute and post-acute care needs (CHF, COPD, wound care, IV therapy, post-surgical care). Triage and independently manage urgent clinical situations in the home setting. Conduct medication reconciliation, patient teaching, and safety monitoring. Complete wound assessments, care, and documentation in the electronic record. 🎯 Requirements Current nursing license in good standing in the state of Pennsylvania. Graduation from an accredited nursing program (transcript or diploma required).
