Licensed Practical Nurse, LPN - Home Health Visits

📋 Description Provide skilled nursing and rehabilitative care in clients\' homes. Travel throughout the office\'s coverage area to support home health visits. Collaborate with a multi-disciplinary team to ensure compassionate, high-quality care. 🎯 Requirements A current nursing license in good standing in the state of Colorado. Graduation from an accredited nursing program. At least one year of hands-on medical experience with adult/geriatric population (recent). Ability to work independently and make real-time clinical decisions in home settings. Strong documentation and time-management skills; flexible to dynamic visit schedules.
