📋 Description Travel daily to provider practices and patient homes for eye exams. Support quality improvement across MA STAR, Commercial, Medicaid, MSSP programs. Identify and close quality gaps; collect data from EMR, claims, payer portals. Complete training on retinal imaging and operate imaging equipment. Educate patients on preventive care and scheduling; support adherence. Manage daily eye exam schedule and travel logistics; mileage covered. 🎯 Requirements Willingness to travel up to 100% to provider practices and patient homes; training provided. Valid driver's license and reliable transportation. Associate degree or higher. 1-3 years of healthcare setting experience. 2 years with MA/Commercial/Medicaid/MSSP quality programs (data abstraction).
