Speech Language Pathologist, SLP - Home Health

📋 Description Make home visits to clients in designated territories. Perform evaluations to diagnose and treat speech, language, or swallowing issues. Develop a treatment plan based on evaluation, orders, and goals. Continually assess and revise care plans with the multidisciplinary team. Accurately document observations and interventions using EMR on a tablet. 🎯 Requirements Active AZ SLP license. ASHA CCC-SLP credential or equivalent. Be pursuing ASHA certification or have completed requirements. Solid computer skills; EMR experience preferred.
