Bilingual Care Coordinator (Pacific Time)

📋 Description Support CCM for patients with CKD and related conditions. Conduct patient outreach and maintain care plans under clinical supervision. Coordinate communication between patients, providers, and partners. Work remotely 8:00am–5:00pm CT; fully remote. Use SMS for patient engagement; phone outreach as needed.
