Medical Social Worker

📋 Description Conduct in-home psychosocial assessments and social work evaluations. Develop individualized intervention plans for clients and families. Assess social, emotional, financial, and environmental factors affecting health outcomes. Connect clients with appropriate community resources and support services. Advocate for clients navigating healthcare and community systems. Collaborate with nurses, therapists, physicians, and other team members. 🎯 Requirements Active New Jersey LSW or LCSW license. MSW from CSWE-accredited program. Minimum of two years of community-based social work experience. EMR experience preferred. Strong communication, interpersonal, and organizational skills.
