Registered Nurse-Home Health Visits

📋 Description

• Provide skilled nursing and rehabilitative care in patients' homes as part of a multi-disciplinary
• Perform comprehensive head-to-toe assessments and identify changes in patient condition
• Manage acute and post-acute conditions (CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and independently handle urgent clinical situations in the home setting
• Conduct medication reconciliation, patient teaching, and safety monitoring
• Document wound assessments and care using point-of-care documentation

🎯 Requirements

• Active nursing license in good standing (Massachusetts)
• Graduation from an accredited nursing program
• At least one year of hands-on experience with adult/geriatric populations in healthcare settings
