Care Coordinator, LSW (MSHO)

📋 Description

• Coordinate face-to-face visits with members
• Manage Elder Waiver and related services
• Conduct annual assessments including PCA tools
• Review health needs and develop individualized care plans
• Connect members with community resources and services
• Support safe transitions of care and follow-up plans

🎯 Requirements

• Current MN Licensed Social Worker (LSW/LMSW/LISW/LICSW) or RN license
• 1+ year geriatric or disabled population experience in care coordination
• Valid driver's license
