📋 Description

• Full-time Admissions Registered Nurse for home health visits
• Serves adult patients in Dover, DE area
• Provides rehabilitative care and skilled nursing at patients' homes
• Works as part of a multi-disciplinary health care team
• Follow care plans, perform meds, wound care, and procedures
• Document observations and assessments, coordinate with clinicians

🎯 Requirements

• Current RN license in DE
• Minimum one year clinical (medical/surgical) experience
• Home care experience preferred
• Graduate from accredited nursing program
• Ability to work independently and manage time
• Strong interpersonal and EMR skills; travel to cases as needed
