Job Description
📋 Description
- Care for WelbeHealth participants in the clinic setting.
- Support clinic operations, monitor vitals, perform point-of-care testing, and administer
- Coordinate with providers on medication orders and changes from procedures.
- Review medications during Welbe Welcome Day and follow post-procedure instructions.
- Escalate questions to RN Case Manager or PCP as needed.
🎯 Requirements
- Graduate of an Accredited School of Nursing with an unencumbered LVN/LPN license.
- Minimum one year nursing experience with frail/elderly populations.
- Strong clinical, organizational, and communication skills with seniors and interdisciplinary teams.
- CPR and First Aid certification within 180 days of hire (preferred at start).
