Job Description
📋 Description
- Outreaches to, intakes, assesses, counsels, and coordinates care for TAY 18-27 across life domains
- Serves as client’s advocate and care coordinator within GLIDE’s continuum of care and community
- Collaborates with TAY HWC team to enable warm hand-offs and linkages among specialists.
- Develops care transition plans to connect clients to additional support services after GLIDE care
- Facilitates and/or coordinates weekly activities or psychoeducation groups to boost confidence and
- May require nontraditional hours; some weekend work may be needed.
🎯 Requirements
- Bachelor’s degree in social work, public health, community health, or related field preferred.
- 5+ years of experience in intensive case management for TAY or similar populations in social
- Knowledge of Bay Area TAY needs/resources; ability to link clients to services and navigate complex
- Strong communication, organizational, and interpersonal skills; ability to work independently and
- Ability to use computer and reporting systems (Windows, MS Office, Salesforce or similar, Zoom).
