Weekend Admissions Physical Therapist

📋 Description

• Complete OASIS Start of Care (SOC) admissions visits for adult/geriatric home health patients.
• Conduct comprehensive PT evaluations: mobility, strength, balance, gait, endurance, transfers
• Develop individualized treatment plans per patient needs and orders.
• Implement interventions to support recovery and independence.
• Collaborate with nursing/interdisciplinary team for coordinated care.
• Educate patients and families on rehab and safe mobility.

🎯 Requirements

• Current New Jersey Physical Therapist license.
• Minimum one year licensed PT experience.
• Home health experience required.
• OASIS experience required.
• HCHB experience strongly preferred.
• Excellent assessment/communication/time management skills.
