Job Description
📋 Description
- OT evaluation and treatment for PACE participants
- Develop individualized treatment plans
- Conduct home visits as needed
- Instruct participants, caregivers, staff on ADLs, exercise, and DME
- Collaborate as core IDT member for care planning
- Document to regulatory standards
🎯 Requirements
- Masters/Doctorate in OT (MOT or DOT) or equivalent
- National Certification by AOTA or eligible for exam
- At least 1 year with frail or elderly
- Spanish/English bilingual strongly preferred
- PACE experience or ADCH/CBAS a plus
- State driver’s license and reliable transportation
