📋 Description

• Clinical lead during enrollment process for Habitat Health's PACE program.
• Conduct eligibility and home safety assessments with IDT collaboration.
• Educate participants and caregivers to support informed enrollment decisions.
• Ensure compliant enrollment workflows and smooth transition into PACE.
• Maintain accurate documentation and regulatory alignment throughout enrollment.
• Build trusting, participant-centered enrollment experiences in homes and centers.

🎯 Requirements

• Active RN license in applicable state; unrestricted.
• Minimum 1 year nursing experience with older adults or complex medical needs.
• Associate's degree in Nursing (required); BSN preferred.
• Experience with NFLOC, home safety assessments, and CMS/regulatory requirements.
• Strong communication with participants, caregivers, providers, and IDT.
• Proficiency with electronic health records and enrollment management systems.
