Job Description
📋 Description
- Coordinate care for patients with kidney disease via an integrated technology platform.
- Provide exceptional patient service and coordination across disciplines with an interdisciplinary
- Lead comprehensive care coordination focusing on Social Determinants of Health.
- Assist patients in navigating the healthcare system and accessing resources.
- Collaborate with Nurse Practitioners, Nurse Care Managers, Dietitians, Pharmacists, social workers
- Support cross-coverage between markets as needed.
🎯 Requirements
- 3 years of experience in health care coordination, community health work, or similar field
- Experience with a team-based approach to care
- Excellent knowledge of community resources
- Strong prioritization and multi-tasking abilities
- Self-directed, professional problem-solving mindset
- Strong communication and teaching skills
