Transitional Care Manager, RN or LPN

📋 Description

• Coordinate initiation of home health care services with discharge planners and physicians
• Develop relationships with referral sources and other health care professionals
• Market and articulate BAYADA services and benefits to clients and facilities
• Review medical documentation and collaborate with physicians on eligibility for home care
• Coordinate services with the office to fulfill physician-ordered care
• Maintain weekly referral log

🎯 Requirements

• Current RN or LPN license in good standing
• Graduate of accredited nursing program
• Strong interpersonal skills
• Knowledge of Medicare COPs and program requirements
• Minimum 1 year experience in healthcare; home health preferred
• Discharge planning experience preferred
