📋 Description

• Coordinate services for vulnerable individuals to help them live in the community.
• Assess, plan, refer, monitor, advocate, and support individuals and guardians in decision making.
• Directly manage a caseload and coordinate other services to positively impact lives.

🎯 Requirements

• Bachelor’s Degree in a human service-related field
• Two years full-time CADI/DD waiver case management experience
• Knowledge of the local community
• Strong oral and written communication skills
• Knowledge of self-directed services concepts
• Valid driver’s license and proof of automobile insurance
