Hospice Care Assistant

📋 Description

• Provide hands-on personal care and compassionate support to hospice patients and families.
• Collaborate with RN case manager and hospice team to support individualized care.
• Follow the patient’s plan of care to ensure comfort, dignity, and respect.
• Maintain a safe, clean, and comfortable home environment for patients.
• Communicate changes in patient condition to the clinical team promptly.
• Support patients’ independence and quality of life through compassionate care.

🎯 Requirements

• CNA/STNA certification in Ohio.
• Minimum one year as a CNA/STNA in hospice or similar setting.
• Valid driver’s license and reliable transportation.
• Clean driving record and current auto insurance.
• Ability to travel within the service area and work independently.
• Strong communication and teamwork skills; patient-focused care.
