Occupational Therapist

📋 Description

• Provide skilled home visits in designated territories
• Evaluate functional abilities (ADLs, IADLs, mobility)
• Create individualized treatment plans
• Assess home env and suggest equipment/modifications
• Collaborate with nurses, therapists, physicians, teams
• Educate patient, family, caregiver on interventions

🎯 Requirements

• Current New Jersey OT license
• Graduate of accredited OT program
• Minimum 1 year OT experience
• Home health experience preferred
• Strong clinical, org, comm, problem-solving
• Ability to work independently and manage time
