Admissions Registered Nurse, Home Health

📋 Description

• Use head-to-toe assessment to identify changes in patient condition
• Provide acute and post-acute care (CHF, COPD, wound care, IV therapy, post-surgical)
• Triage and manage urgent clinical situations in the home
• Perform medication reconciliation, teaching, safety monitoring
• Conduct comprehensive wound assessment, care, and documentation

🎯 Requirements

• Current nursing license in good standing in the state of Pennsylvania
• Graduation from an accredited and approved nursing program
• At least one year of hands-on medical experience within the past two years with adult/geriatric
