Speech Language Pathologist-Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess, diagnose, and provide therapy for speech, language, or swallowing
• Develop treatment plans based on evaluations and goals.
• Revise care plans with multidisciplinary BAYADA team as appropriate.
• Educate patients and families on rehabilitative care and activities for health and independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• Current PA Speech Language Pathologist license.
• CCC-SLP or equivalent certification/experience pathway; ASHA prerequisites acceptable.
• Experience with EMR systems preferred.
