Speech Language Pathologist, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients
• Develop a treatment plan based on the evaluation, physician’s orders, and goals.
• Continuously assess and revise the speech therapy care plan with the multidisciplinary team.
• Educate patients and families in rehabilitative care to promote health and independent living.
• Document observations and interventions using a state-of-the-art touch pad tablet.

🎯 Requirements

• Current Indiana licensure as a Speech Language Pathologist.
• Certificate of Clinical Competence in Speech (CCC-SLP) from ASHA, or equivalent credentials or
• Educational background/experience requirements for certification; in process of obtaining
• Solid computer skills; prior EMR experience preferred.
