Registered Nurse, Home Health

📋 Description

• Visit clients in their own homes for recovery support
• Provide 1:1 direct nursing care in home settings
• Case manage, develop care plans, and deliver skilled care
• Assessments, medication management, wound care
• Progress clients from discharge to goals at home
• Educate patients and families on care plans

🎯 Requirements

• Active and current RN license
• One year of clinical experience preferred
• Home health experience welcomed but not required
• Ability to travel locally to cases
