Home Health Speech Language Pathologist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations for speech, language, communication, or swallowing issues.
• Develop treatment plans based on evaluations and physician orders.
• Collaborate with BAYADA clinicians in multidisciplinary care plans.
• Educate patients and families on rehabilitative care for independence.
• Document observations and interventions in EMR on a touch pad device.

🎯 Requirements

• Current MD Speech Language Pathologist license.
• ASHA CCC or equivalent, or nearing certification.
• Completed education/work requirements for certification.
• Solid computer skills; EMR experience preferred.
