Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, transfer status, gait, strength, balance
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy, gait/balance training, and

🎯 Requirements

• Current Pennsylvania Physical Therapist license.
• Minimum one year of recent PT clinical experience.
• Graduate from an APTA/CAAHEA-accredited program or equivalent (per transcript/diploma).
• Ability to work independently and manage time; strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.
