Home Health Visits Registered Nurse

📋 Description

• Perform head-to-toe assessments and identify subtle changes in patient condition
• Apply acute and post-acute care skills (CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and independently manage urgent clinical situations in the home
• Conduct medication reconciliation, patient teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation

🎯 Requirements

• A current nursing license in good standing in the state of Massachusetts
• Graduation from an accredited nursing program
• At least one year of current or recent hands-on medical experience with adult/geriatric population
