Care Manager, RN (Lorain OH / PART-TIME / Field-based)

📋 Description Conduct initial in-home assessments and orientations for patients and caregivers. Provide ongoing care management and coordination with interdisciplinary teams. Educate patients, PCAs, and families; ensure quality assurance and regulatory compliance. Coordinate service activation, care plans, and ongoing monitoring through home visits. Maintain documentation and communicate clinical concerns for timely intervention. Ensure continuity of care and adherence to agency policies. 🎯 Requirements Registered Nurse (RN) with valid license in Ohio. Care coordination and in-home case management experience preferred. Proficiency with MS Office and Case Management Systems; ability to teach others. Strong communication, empathy, and patient-centered approach. Ability to travel within assigned geographic area; valid driver’s license and car insurance.
