Licensed Practical Nurse (LPN)- Home Health Visits

📋 Description Provide skilled nursing and rehabilitative care to adult clients in their homes on a per-visit Make home visits to clients in designated territories in Dauphin and Cumberland counties. Administer medications, wound care, treatments, and procedures per physician orders. Monitor client conditions and report changes to Clinical Manager and Client Services Manager. Document observations and interventions in electronic medical records. Collaborate with a multidisciplinary health care team to support home-based care. 🎯 Requirements One year of clinical experience as a Licensed Practical Nurse (LPN) required. Current LPN license in Pennsylvania. Graduate from an accredited nursing program; strong EMR skills preferred. Ability to travel to cases as assigned; independent time management; strong interpersonal skills.
