📋 Description Make home visits to clients in designated geographic territories. Perform evaluations to assess and diagnose, provide interventions for speech, language Develop treatment plans based on evaluation, physician orders and goals. Revise the care plan and participate in multidisciplinary team discussions as appropriate. Educate patients and families in rehabilitative care to promote health and independence. Document observations and interventions using a touch pad device. 🎯 Requirements Current Rhode Island Speech-Language Pathologist license.
