Medical Social Worker, (MSW) - Home Health

📋 Description Make home visits to clients in designated geographic territories. Assess social and emotional factors related to patients' health issues. Provide clinical social evaluations and plan interventions based on findings. Identify community resources to address patient and family needs. Advocate for clients and families to navigate community systems. Collaborate with the multidisciplinary BAYADA team to develop care plans. 🎯 Requirements Minimum of a Master’s in Social Work with at least 1 year of health care experience. Council on Social Work Education accreditation demonstrated by transcript/degree. Home care experience a plus; able to read, write, and communicate in English. Ability to work independently and manage time; strong interpersonal skills. Solid computer skills; prior EMR experience preferred.
